MultiMarkdown version 3.0a4 released

Version 3.0a4 fixes a bug or two, add support for width and height attributes in LaTeX output, and adds support for “locators” in citations in XHTML and LaTeX output.
The output syntax varies slightly from MultiMarkdown version 2. But I think all of the major features are now implemented, with the exception of support for ASCIIMathML. I haven’t entirely decided what I’m going to do about that.
